The #2 reason is attributed to having a problematic business plan from the start. Coincidentally these statistic have not change much since 1995.

One of the most common opportunities we see for growth and improvement in business is in two areas; delegation of responsibilities and communication. In most situations it’s not poor communication, and it’s not usually lack of communication, it’s what’s being conveyed and how it’s being received.
An open minded conversation is sometimes all it takes to start getting immediate and improved results, but it takes far more to consistently and reliably maintain these improvements.
Additionally, delegating responsibilities through effective training and clear expectations is the fastest way to increase profit and decrease workload. You must empower others to do parts of your job, until you give your job away entirely. Then you can be free.
